About this trial
Investigators will evaluate the effectiveness of two different EHR-embedded nudges to improve referral to cardiac rehabilitation.
Eligibility criteria
Qualifiers
Age of at least 18 years
History of Coronary Artery Bypass Grafting (CABG), Percutaneous Coronary Intervention (PCI), or valve repair/replacement in the past year
Disqualifiers
Hospice
Active home health
