Methods that accelerate tooth movement may reduce overall treatment time and potentially influence the extent of root resorption.\n\nCorticotomy-assisted orthodontics is based on the regional acceleratory phenomenon, which enhances bone remodeling and facilitates tooth movement. Modified corticotomy techniques involve limited surgical injury to the cortical bone to stimulate accelerated movement while minimizing postoperative morbidity. Frictionless mechanics using T-loops allow controlled canine retraction with reduced frictional resistance between the bracket and archwire.\n\nThis single-center randomized controlled clinical trial will enroll 60 patients aged 12 to 30 years who require bilateral extraction of maxillary first premolars as part of orthodontic treatment. Participants will be randomly allocated in a 1:1 ratio to:\n\nGroup A: Modified corticotomy-assisted canine retraction with frictionless T-loop mechanics Group B: Conventional frictionless T-loop canine retraction\n\nAll participants will be treated using 0.022 × 0.028-inch preadjusted edgewise appliances. Standardized T-loop activation delivering controlled force will be performed at three-week intervals until canine retraction is completed.\n\nExternal apical root resorption will be assessed by measuring linear changes in root length from the canine cusp tip to the root apex on calibrated pre- and post-retraction panoramic radiographs using digital measurement software. The primary outcome measure will be the mean change in root length between baseline and post-retraction assessments. Statistical analysis will compare the magnitude of root resorption between the two groups.",[36,42],{"label":37,"type":38,"description":39,"interventionNames":40},"Modified Corticotomy Assisted Canine Retraction","EXPERIMENTAL","Description: Participants will undergo modified corticotomy procedure followed by canine retraction using frictionless T-loop mechanics after extraction of maxillary first premolars.\n\nIntervention:\n\n* Procedure: Modified Corticotomy\n* Procedure: Frictionless T-loop canine retraction.",[41],"Procedure: Modified Corticotomy assisted frictionless canine retraction",{"label":43,"type":44,"description":45,"interventionNames":46},"Conventional T-loop Canine Retraction","ACTIVE_COMPARATOR","Description: Participants will undergo conventional canine retraction using frictionless T-loop mechanics without any surgical intervention after extraction of maxillary first premolars.\n\nIntervention: Frictionless T-loop canine retraction.",[47],"Procedure: Conventional Frictionless Canine retraction",[49,54],{"type":50,"name":51,"description":52,"armGroupLabels":53,"otherNames":28},"PROCEDURE","Modified Corticotomy assisted frictionless canine retraction","A surgical procedure in which a buccal flap is elevated and two vertical corticotomy cuts are made in the maxillary cortical bone adjacent to the extraction site to accelerate orthodontic tooth movement.",[37],{"type":50,"name":55,"description":56,"armGroupLabels":57,"otherNames":28},"Conventional Frictionless Canine retraction","Orthodontic space closure using 17×25 TMA T-loops activated to deliver approximately 200 g of force for frictionless canine retraction into maxillary first premolar extraction spaces.",[43],[59],{"name":60,"affiliation":61,"role":62},"Prof. Dr. Haroon Shahid Qazi, BDS, MS, MCPS (HPE)","Islamabad Medical & Dental Hospital","STUDY_CHAIR",[64,69],{"name":65,"role":66,"phone":67,"phoneExt":28,"email":68},"Dr. Menal Khurshid, BDS","CONTACT","0333-5303397","menal.15@iideas.edu.pk",{"name":70,"role":66,"phone":71,"phoneExt":28,"email":72},"Prof. Dr. Haroon Shahid Qazi, BDS, MS, MCPS (HPE),","0300-5001774","haroon77@gmail.com",[74],{"facility":75,"status":28,"city":76,"state":77,"zip":78,"country":79,"countryCode":80,"cosmosGeoPoint":81,"geoPoint":86,"contacts":87},"Islamabad Dental Hospital","Islamabad","Federal","45720","Pakistan","PK",{"type":82,"coordinates":83},"Point",[84,85],73.04329,33.72148,{"lat":85,"lon":84},[88],{"name":89,"role":66,"phone":67,"phoneExt":28,"email":68},"Menal Khurshid, BDS",{"type":91,"investigatorFullName":28,"investigatorTitle":28,"investigatorAffiliation":28,"oldNameTitle":28,"oldOrganization":28},"SPONSOR","100628169",false,"NCT07458139","Comparison of Root Resorption in Modified Corticotomy-Assisted and Conventional Canine Retraction","Comparison of External Apical Root Resorption in Modified Corticotomy-Assisted and Conventionally Retracted Maxillary Canines Using Frictionless T-Loop Mechanics: A Randomized Controlled Clinical Trial","MCA-CR","Inclusion Criteria:\n\n* Patients aged 12-30 years requiring canine retraction into maxillary first premolar extraction spaces\n* Patients with normal vertical skeletal pattern (mandibular plane angle 32° ± 5°)\n* Patients with healthy periodontium\n\nExclusion Criteria:\n\n* Syndromic patients\n* Patients with systemic conditions requiring long-term medication affecting orthodontic tooth movement (e.g., NSAIDs, bisphosphonates, anticonvulsants)\n* Mixed dentition cases\n* Patients with pre-existing root resorption\n* Patients who fail to attend scheduled appointments",true,"ALL","12 Years","30 Years",{"count":104,"type":105},60,"ESTIMATED","INTERVENTIONAL",[108],"NA","This randomized controlled clinical trial will compare external apical root resorption in patients undergoing maxillary canine retraction using modified corticotomy-assisted frictionless T-loop mechanics versus conventional frictionless T-loop mechanics.
