While subsequent development of type II diabetes elevates this risk, prior GDM is an independent risk factor for CVD morbidity, particularly within the first decade postpartum. GDM is associated with impaired endothelial function during pregnancy and decrements in macro- and microvascular function persist postpartum, despite the remission of insulin resistance following delivery. Collectively, while the association between GDM and elevated lifetime CVD risk is clear, and available evidence demonstrates a link between GDM and vascular dysfunction in the decade following pregnancy, the mechanisms mediating this persistent dysfunction remain unexamined.\n\nThe purpose of this investigation is to examine NADPH oxidase as a source of reactive oxygen species contributing to aberrant microvascular function in otherwise healthy women with a history of GDM.\n\nIn this study, the investigators use the blood vessels in the skin as a representative vascular bed for examining mechanisms of microvascular dysfunction in humans. Using a minimally invasive technique (intradermal microdialysis for the local delivery of pharmaceutical agents) they examine the blood vessels in a dime-sized area of the skin in women who have had GDM. As a compliment to these measures, the investigators also collect endothelial cells from an antecubital vein and measure NADPH oxidase expression and markers of oxidative stress in these cells.",[29,36,41,45],{"label":30,"type":31,"description":32,"interventionNames":33},"local lactated Ringer's perfusion","PLACEBO_COMPARATOR","lactated Ringer's is perfused through the microdialysis fiber to serve as the vehicle control",[34,35],"Drug: Acetylcholine","Drug: Insulin aspart",{"label":37,"type":38,"description":39,"interventionNames":40},"local apocynin perfusion","EXPERIMENTAL","local apocynin is perfused through the microdialysis fiber to serve as the NADPH oxidase inhibited experimental treatment",[34,35],{"label":42,"type":38,"description":43,"interventionNames":44},"local L-NAME perfusion","local L-NAME is perfused through the microdialysis fiber to inhibit nitric oxide synthase",[34,35],{"label":46,"type":38,"description":47,"interventionNames":48},"local apocynin + L-NAME perfusion","local apocynin and L-NAME are perfused through the microdialysis fiber for dual inhibition of NADPH oxidase and nitric oxide synthase",[34,35],[50,55],{"type":51,"name":52,"description":53,"armGroupLabels":54,"otherNames":20},"DRUG","Acetylcholine","acetylcholine is perfused at 10 ascending concentrations (10\\^-10M - 10\\^-1 M) for 5 minutes each",[42,46,37,30],{"type":51,"name":56,"description":57,"armGroupLabels":58,"otherNames":20},"Insulin aspart","insulin aspart is perfused at 5 ascending concentrations (10\\^-8M - 10\\^-4 M) for 10 minutes each",[42,46,37,30],[60],{"name":61,"role":62,"phone":63,"phoneExt":20,"email":64},"Anna Reid-Stanhewicz, PHD","CONTACT","319-467-1732","anna-stanhewicz@uiowa.edu",[66],{"facility":5,"status":67,"city":68,"state":69,"zip":70,"country":71,"countryCode":72,"cosmosGeoPoint":73,"geoPoint":78,"contacts":79},"RECRUITING","Iowa City","Iowa","52242","United States","US",{"type":74,"coordinates":75},"Point",[76,77],-91.53017,41.66113,{"lat":77,"lon":76},[80],{"name":81,"role":62,"phone":63,"phoneExt":20,"email":64},"Anna Stanhewicz, PhD",{"type":83,"investigatorFullName":81,"investigatorTitle":84,"investigatorAffiliation":5,"oldNameTitle":20,"oldOrganization":20},"SPONSOR_INVESTIGATOR","Assistant Professor","100512003",false,"NCT05946798","Role of NADPH Oxidase in Microvascular Dysfunction Following GDM","INCLUSION CRITERIA:\n\n* 18 years or older\n* pregnant within 5 years of the study visit\n* had gestational diabetes diagnosed by their obstetrician and confirmed according to the American College of Obstetricians and Gynecologists criteria for gestational diabetes.\n* or without a history of gestational diabetes\n\nEXCLUSION CRITERIA:\n\n* skin diseases\n* current tobacco or electronic cigarette\u002Fvape pen use,\n* diagnosed or suspected hepatic or metabolic disease including diabetes,\n* statin or other cholesterol-lowering medication,\n* current antihypertensive medication,\n* history of preeclampsia or gestational hypertension,\n* current pregnancy,\n* body mass index \\\u003C18.5 kg\u002Fm2,\n* allergy to materials used during the experiment.(e.g. latex),\n* known allergies to study drugs",true,"FEMALE","18 Years","50 Years",{"count":95,"type":96},40,"ESTIMATED","INTERVENTIONAL",[99],"EARLY_PHASE1","The purpose of this investigation is to examine NADPH oxidase as a source of reactive oxygen species contributing to aberrant microvascular function in otherwise healthy women with a history of GDM.",[102,103,104],"Gestational Diabetes","Oxidative Stress","Vascular Endothelial Function",[106,107,108],"gestational diabetes","postpartum","vascular","2025-05-07",{"date":111,"type":112},"2025-05-13","ACTUAL",{"date":114,"type":112},"2023-08-30",{"date":116,"type":96},"2028-06-01",{"name":81,"class":6},1]
