Results from randomised trials (CRASH-3 and NCT01990768) show that early treatment with TXA (given intravenously) reduces head injury deaths (pooled RR 0.89, 95% CI 0.80-0.99). In the CRASH-3 trial, the reduction in head injury deaths with TXA was largest in patients with mild and moderate head injuries, particularly if patients were treated soon after injury. However, the CRASH-3 trial included mild TBI patients only if they had intracranial bleeding on CT scan. It is uncertain whether the results apply to mild TBI patients more generally. CRASH-4 is a randomised, double blind, placebo-controlled trial in symptomatic mild TBI in about 10,000 older adults. The pilot phase will include about 500 patients. The trial aims to provide reliable evidence about the effects of early intramuscular TXA on intracranial haemorrhage, disability, death, and dementia in older adults with symptomatic mild head injury.",[76,82],{"label":77,"type":78,"description":79,"interventionNames":80},"Active","ACTIVE_COMPARATOR","A single dose of Tranexamic acid 500mg given by intramuscular injection",[81],"Drug: Tranexamic Acid 500 MG",{"label":83,"type":84,"description":85,"interventionNames":86},"Placebo","PLACEBO_COMPARATOR","One Injection of the placebo which is 10 mL Sodium Chloride (0.9%)",[81],[88],{"type":89,"name":90,"description":91,"armGroupLabels":92,"otherNames":45},"DRUG","Tranexamic Acid 500 MG","given once as an intramuscular injection",[77,83],[94,97],{"name":95,"affiliation":5,"role":96},"Haleema Shakur-Still","STUDY_CHAIR",{"name":98,"affiliation":5,"role":96},"Ian Roberts",[100],{"name":95,"role":101,"phone":102,"phoneExt":45,"email":103},"CONTACT","+4407714139500","Haleema.Shakur@lshtm.ac.uk",[105,121],{"facility":106,"status":107,"city":108,"state":108,"zip":109,"country":110,"countryCode":111,"cosmosGeoPoint":112,"geoPoint":117,"contacts":118},"The Royal London Hospital","RECRUITING","London","E1 1FR","United Kingdom","UK",{"type":113,"coordinates":114},"Point",[115,116],-0.12574,51.50853,{"lat":116,"lon":115},[119],{"name":120,"role":101,"phone":45,"phoneExt":45,"email":45},"Ben Bloom",{"facility":122,"status":107,"city":108,"state":108,"zip":123,"country":110,"countryCode":111,"cosmosGeoPoint":124,"geoPoint":126,"contacts":127},"St George's Hospital","SW17 0QT",{"type":113,"coordinates":125},[115,116],{"lat":116,"lon":115},[128],{"name":129,"role":101,"phone":45,"phoneExt":45,"email":45},"Heather Jarman",{"type":131,"investigatorFullName":45,"investigatorTitle":45,"investigatorAffiliation":45,"oldNameTitle":45,"oldOrganization":45},"SPONSOR","100402562",false,"NCT04521881","Clinical Randomisation of an Anti-fibrinolytic in Symptomatic Mild Head Injury in Older Adults","Intramuscular Tranexamic Acid for the Treatment of Symptomatic Mild Traumatic Brain Injury in Older Adults: a Randomised, Double-blind, Placebo-controlled Trial","CRASH-4","Inclusion Criteria:\n\n* 50 years or older (actual or estimated)\n* History or evidence of head injury (e.g. laceration, bruise, swelling or pain in head or face)\n* GCS ≥ 13\n* Has one or more of the following:\n\n 1. has or had any impaired consciousness (loss of consciousness, amnesia, or confusion)\n 2. nausea or vomiting\n* Within 3 hours of injury (do not include if interval cannot be estimated e.g. patient unable to confirm time of fall or patient found on floor after an unwitnessed fall and home alone)\n* Not living in a nursing home, mental health institution or prison\n* Patient will be conveyed to or is admitted to a participating hospital\n\nExclusion Criteria:\n\n\\- TXA not clearly indicated (e.g. major bleeding) or contraindicated (e.g. suspected stroke)","ALL","50 Years",{"count":142,"type":143},5000,"ESTIMATED","INTERVENTIONAL",[146],"PHASE3","Tranexamic acid (TXA) reduces head injury deaths.
