Automated screening will occur of over 12 million patients in England, with targeted recruitment to practices with eligible patients, regular updates to General Practitioners, simple processes for centre inclusion and patient randomisation, remote e-consent and no additional visits for any patient. The primary outcome is a comprehensive composite of any thromboembolic event, ascertained entirely using electronic healthcare records within both primary and secondary NHS care across the nation. All endpoint data will follow a pre-published coding manual for extracted electronic healthcare data. The key secondary outcome is the change in patient-reported cognitive function, using remote technology solutions to save time for clinical staff and patients. DaRe2THINK will carefully assess and validate safety outcomes relating to major and minor bleeding. A systematic health economic analysis will determine NHS and societal cost-effectiveness of DOAC therapy in this younger population of patients with AF. DaRe2THINK will initially run over a 5-year period (outcomes as listed below), with longer-term outcomes (in particular cardiovascular death, cognitive function and vascular dementia) reassessed at 10 years.",[106,112],{"label":107,"type":108,"description":109,"interventionNames":110},"Direct Oral anticoagulants (DOAC)","EXPERIMENTAL","Commence DOAC even with low or intermediate risk of stroke or thromboembolism, which could include currently licensed drugs apixaban, dabigatran, edoxaban or rivaroxaban; choice of drug and dose according to local practice guidelines",[111],"Drug: Direct Oral Anticoagulants",{"label":113,"type":114,"description":115,"interventionNames":101},"No anticoagulant therapy (usual care)","NO_INTERVENTION","Continuation of usual anticoagulant prescribing practice in patients with AF; e.g. according to National Institute for Health and Care Excellence (NICE), patients with AF should commence oral anticoagulation with a CHA2DS2-VASc score of 2 or above.",[117],{"type":118,"name":119,"description":120,"armGroupLabels":121,"otherNames":122},"DRUG","Direct Oral Anticoagulants","choice of DOAC (apixaban, dabigatran, edoxaban or rivaroxaban) according to local practice",[107],[123],"apixaban, dabigatran, edoxaban or rivaroxaban",[125,129,133,136],{"name":126,"affiliation":127,"role":128},"Dipak Kotecha","University of Birmingham and University Hospitals Birmingham NHS Foundation Trust","PRINCIPAL_INVESTIGATOR",{"name":130,"affiliation":131,"role":132},"John Camm","St George's University of London; Chair of DaRe2THINK Independent TSC","STUDY_CHAIR",{"name":134,"affiliation":135,"role":132},"Marcus Flather","Norwich Medical School; Chaire of DaRe2THINK Independent DMC",{"name":137,"affiliation":138,"role":128},"David Shukla","Deputy CI; Lead for NIHR West Midlands Primary Care CRN Team",[140,145],{"name":141,"role":142,"phone":143,"phoneExt":101,"email":144},"Alastair Mobley, BSc","CONTACT","+44 121 371 4225","a.mobley@bham.ac.uk",{"name":126,"role":142,"phone":143,"phoneExt":101,"email":146},"d.kotecha@bham.ac.uk",[148],{"facility":149,"status":150,"city":151,"state":152,"zip":153,"country":154,"countryCode":155,"cosmosGeoPoint":156,"geoPoint":161,"contacts":162},"University Hospitals Birmingham","RECRUITING","Birmingham","West Midlands","B15 2TH","United Kingdom","UK",{"type":157,"coordinates":158},"Point",[159,160],-1.89983,52.48142,{"lat":160,"lon":159},[163],{"name":164,"role":142,"phone":165,"phoneExt":101,"email":166},"Minnie Ventura","0121 371 8145","Maximina.Ventura@uhb.nhs.uk",{"type":168,"investigatorFullName":101,"investigatorTitle":101,"investigatorAffiliation":101,"oldNameTitle":101,"oldOrganization":101},"SPONSOR",[170,173,175,177,179],{"name":171,"class":172},"Clinical Practice Research Datalink","OTHER_GOV",{"name":174,"class":6},"University Hospital Birmingham NHS Foundation Trust",{"name":176,"class":6},"University of Oxford",{"name":178,"class":6},"London School of Economics and Political Science",{"name":180,"class":6},"Aston University","100416297",false,"NCT04700826","Preventing Stroke, Premature Death and Cognitive Decline in a Broader Community of Patients With Atrial Fibrillation","Preventing Stroke, Premature Death and Cognitive Decline in a Broader Community of Patients With Atrial Fibrillation Using Healthcare Data for Pragmatic Research: A Randomised Controlled Trial","DaRe2THINK","Inclusion Criteria:\n\n1. The trial will test the hypothesis that direct oral anticoagulants (DOACs), now commonly used in older patients with atrial fibrillation (AF), are effective and cost-effective at reducing major adverse clinical events in younger patients at low or intermediate risk of stroke, and can reduce the high rate of cognitive decline. The health technology innovations noted above will allow the investigators to answer this important clinical question, as well as demonstrate the capacity and potential of this system for future, large-scale healthcare-embedded clinical trials for patient benefit.",[199],"Atrial Fibrillation",[201,202,203,204],"Atrial fibrillation","anticoagulation","stroke","dementia","2025-05-13",{"date":207,"type":208},"2025-05-16","ACTUAL",{"date":210,"type":208},"2021-06-01",{"date":212,"type":193},"2031-01",{"name":5,"class":6},1]
