This is defined as ever having been diagnosed with a medical or psychiatric chronic condition, or currently taking prescription medications for a chronic condition.\n* able to read, understand, and speak English.\n\nExclusion Criteria:\n\n* ineligible for WI Medicaid\n* no substance use history\n* not living with, or no history of, chronic medical or psychiatric condition","ALL","18 Years",{"count":19,"type":20},488,"ESTIMATED","INTERVENTIONAL",[23],"NA","The goal of this clinical trial is to learn if a short program for people being released from prison can help connect them to medical care.
